Concealed Carry Online Application Information

To be considered for an Arizona Concealed Weapon Permit (CCW), you must fully and accurately complete this application. Any false information provided will be considered a criminal offense and will result in the denial of your CCW application.


Before you begin the application process, it’s essential that you review and familiarize yourself with Arizona Revised Statutes Title 13, Chapters 4 and 31. You will be required to confirm that you have read and understood these laws as part of your application.


Eligibility Requirements:

• You must be at least 18 years old to apply for an Arizona CCW permit.

• Active-duty military personnel are eligible to apply starting at age 19.


Additional Concealed Weapon Permit Requirements: must meet ALL requirements

• All new CCW applicants must be fingerprinted as part of the application process.


• You have two options for fingerprinting:

    Option 1 - Electronic Fingerprints via FieldPrint: 

Once you have submitted your application online, you will need to schedule an appointment to have your fingerprints taken at FieldPrint.  As part of scheduling your FieldPrint appointment, you will need to provide the Order Number from the submitted CCW application.


Once your fingerprints have been taken, the Department of Public Safety will then process your CCW application.


     Option 2 - Physical Fingerprint Cards:

You must go to a law enforcement agency or a fingerprint provider to obtain TWO (2) fingerprint cards.  The cards must:

• Fingerprints shall be taken by a qualified fingerprint technician.

• All boxes must be filled in.


IF YOU ARE PROVIDING HARD CARD FINGERPRINTS, YOU MUST INCLUDE A COVERSHEET THAT INCLUDES THE ORDER NUMBER OF YOUR APPLICATION.  YOU CAN PRINT THIS FORM ONCE YOU HAVE SUBMITTED THE CCW APPLICATION.


The supporting documents you will need to provide as part of your Concealed Weapons Permit application:


NEW Concealed Weapon Permit:

Demonstrated competence with a firearm:

• Submit adequate documentation that you have satisfactorily completed a training program or demonstrated competence with a firearm in any state or political subdivision in the United States under A.R.S. §31-3112§(6)(a) through (d) or (N)(1) through (8).


Proof of Identity:

• If born outside the United States or one of its territories, submit a copy of one of the following:

       - Record of birth abroad to an American citizen.

       - Record of birth to Armed Service personnel.

       - Passport issued by the United States

       - Certificate of Naturalization


If not a citizen of the United States, you must be a resident of Arizona:

• Conditional Residents do not qualify for an AZ Permit. 

• Submit a copy of each of the following:

       - A permanent resident alien card (front and back), USCIS Form I-94, or other federally issued document authorizing the applicant to be in the United States.

       - Copy of a resident alien card must have the following:

               • Clearly visible “A” number.

               • The issue and expiration dates must be imprinted on the front of the card.

               • The card must be current for the entire duration of CCW permit issuance.

       - Proof of Arizona residency (as defined by A.R.S. §28-2001):

               • Arizona Driver License or ID Card matching the address on the application, OR:

               • Two documents issued from separate businesses, organizations, or government agencies (utility bills, credit card/bank statements, insurance policy, lease, etc.) with your legal name, as shown on your AZ DL or ID Card, and physical address listed (not a PO Box), matching the address provided on your application.


If you are applying from outside the state of Arizona:

• Two (2) Fingerprint Cards: 

       - Fingerprints shall be taken by a qualified fingerprint technician.

       - All boxes must be filled in.

       - It is recommended you contact your local law enforcement agency or professional fingerprinting service.


There is a $65 fee associated with this application.

You will also need a proof of Identity:

• Government-issued photo identification (ID) such as:

       - A driver license

       - State-issued identification card

       - Passport

• If born outside the United States or one of its territories, submit a copy of one of the following:

       - Record of birth abroad to an American citizen.

       - Record of birth to Armed Service personnel.

       - Passport issued by the United States

       - Certificate of Naturalization


If not a citizen of the United States, you must be a resident of Arizona.

• Proof of address change

• Government-issued photo identification (ID) such as:

       - A driver licenses

       - State-issued identification card


You will need to provide the following for a name change:

The permit holder shall ensure that the written request for a revised Concealed Weapons Permit:

• Contains both the previous and new names,

• Is accompanied by a copy of the court document or marriage certificate authorizing the name change.
