NYC PISTOL PERMIT ADDITIONAL INFORMAITON REQUIRED
- Training certificate
- One copy of your social security card
- One copy of your passport or your birth certificate
- Four character letter (notarized)(Sample Template)
- One copy of your utility bill (even if it's not in the applicant's name)
- One copy of your driver license
- One passport picture
- Acknowledgement of Person Agreeing to Safeguard Firearm(s)
- One Copy of Cohabitant (The Cohabitant is for everyone that is 18yrs or older who lives with you)
- Affirmation of Familiarity
- Understanding of Penal Law
- Social Media Handle
- One DMV abstract of LIFETIME driving record
Please go to the NY DMV website and search “Get My Own Driving Record Abstract.” Log in through NY.gov/MyDMV, choose Driving Record Abstract, then select Lifetime Abstract. Pay the DMV fee and download the PDF. Please save the PDF right away, because DMV only keeps it available online for a short time after purchase.
(Here are the current NY DMV online steps to get a Lifetime Abstract of Driving Record:
- Go to the NY DMV page: “Get My Own Driving Record (Abstract)”
- Choose Order Your Driver Abstract / MyDMV.
- Sign in with your NY.gov ID, or create one. You may need:
- NYS Client ID number
- Document number from your most recent NY license/permit/non-driver ID
- Date of birth
- ZIP code on DMV record
- Last 4 digits of SSN
- Select Driving Record Abstract.
- When asked what type of abstract you want, choose Lifetime.
- Pay the fee online. NY DMV currently lists the online fee as $7.
- Download and save/print the PDF immediately. NY DMV says the abstract remains available in MyDMV for only 5 days after purchase and shows the record as of the time it was ordered.
Important: If the person does not have a current or most recent NYS license, permit, or non-driver ID, they may need to use the Records Request Navigator instead. DMV says Navigator requests can take about 5 business days to process)
